Snag
Photograph the defect and add a note on site. It's logged before you've walked to the next room.

Hand the snag to a person. They're the assignee, so it's their problem to own until it's signed off, and everyone can see whose it is.

Every reply, photo and status change lands on the snag itself. No matching loose messages up, no copying updates into the list by hand. It stays current right up to sign off.

Whoever you hand a defect to gets an email straight away. They can reply to it, and their comments and photos arrive as a reply on the snag itself, filed against the defect instead of sitting in your inbox waiting to be copied across. A free account is made for them the first time you assign them work, so the app is there when they want it.

An assignee never occupies a seat. What you pay for is the people running the job, not the people fixing it.

Only their own work
They see the snags handed to them and the projects those sit in, and nothing else on the job. Opening one in the app, they add a comment, attach photos and move it on from there.

An assignee can take a snag as far as resolved. Closing it is your organisation's call, so nothing leaves the list on the word of the person who did the work.
